Insurance Plans We Accept

We are currently in-network with:

✓ Blue Cross Blue Shield PPO
✓ UnitedHealthcare / Optum

Please note that insurance participation may vary by clinician.

Even when a therapist is in-network, your individual cost depends on your specific insurance plan and may include:
Deductibles
Copays
Coinsurance

Because every plan is different, we recommend verifying your behavioral health benefits before your first appointment.

How to check your insurance benefits

If you'd like to better understand your expected costs before scheduling, call the Member Services number on the back of your insurance card and ask about your outpatient behavioral health benefits.

Helpful questions include:

Is Embodied Healing Counseling and Yoga in-network with my plan?
Is my therapist in-network?
Do I have a deductible for outpatient mental health services?
How much of my deductible has been met?
What will I owe for each therapy session?
Do I have a copay or coinsurance?
Are telehealth sessions covered?
Do I need a referral or prior authorization?
Is there a limit to the number of therapy sessions covered each year?

Our commonly used billing codes are:
90791 — Initial diagnostic evaluation (intake)
90837 — Individual psychotherapy (53+ minutes)

Please remember that any information provided by your insurance company is only an estimate of benefits and is not a guarantee of payment.

Self-pay therapy

Many clients choose not to use insurance, either because they prefer greater privacy, have an out-of-network plan, or simply want the flexibility of paying directly for services.

Our self-pay rates are:

Initial Intake Appointment: $170

Individual Psychotherapy: $150

Payment is due at the time of your appointment and we require a credit card, HSA, or FSA card on file before your first appointment.

Out-of-network benefits

If we are not in-network with your insurance plan, you may still be able to receive reimbursement through your out-of-network mental health benefits.
We are happy to provide a superbill, which is a detailed receipt you can submit to your insurance company for possible reimbursement.

If you plan to use out-of-network benefits, ask your insurance company:

- Do I have out-of-network mental health benefits?
- What percentage of my sessions will be reimbursed?
- What is my out-of-network deductible?
- How do I submit a superbill?

Reimbursement varies by insurance plan.

Reduced-rate appointments

We believe that finances should not automatically prevent someone from accessing quality mental health care.

A limited number of reduced-rate appointments are available based on clinician availability and financial need.

If cost is a concern, please reach out. We're happy to discuss whether a reduced-rate appointment may be available or explore other resources that might be a good fit.

*Note that we are unable to offer reduced rates if you are also using insurance.

Cancellation & No-Show Policy

Your appointment time is reserved specifically for you.

If you need to cancel or reschedule, we ask for at least 24 hours' notice.

Appointments canceled with less than 24 hours' notice, as well as missed appointments, are charged a $100 fee.

Providing advance notice allows your therapist to offer that appointment time to another client who may be waiting for care and helps us honor the time reserved specifically for your session.

Frequently Asked Questions

Do you verify my insurance benefits?

We can verify your eligibility and benefits as a courtesy, but we always recommend confirming your coverage directly with your insurance company. Ultimately, clients are responsible for any costs not covered by their insurance plan.

What if my deductible hasn't been met?

If your deductible has not been met, your insurance company may apply the full contracted session fee toward your deductible until it has been satisfied.

Can I use my HSA or FSA card?

In most cases, yes. Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) can typically be used for psychotherapy services. We recommend confirming eligibility with your plan administrator.

What if you aren't in-network with my insurance?

If we're out-of-network with your plan, you may still be eligible for reimbursement through your out-of-network benefits. We can provide a superbill for you to submit to your insurance company.

Do you offer virtual therapy?

Yes. We offer both in-person therapy in Evanston and virtual therapy throughout Illinois.
